What a hip X-ray shows
A bone X-ray uses ionizing radiation to create pictures of skeletal structures. On the image, dense bone appears white, softer tissue looks gray, and air appears black. For the hip, this makes an X-ray very good at revealing the shape and integrity of the ball-and-socket joint where the femoral head meets the pelvis.
A hip X-ray can help show fractures, dislocations, joint alignment, arthritis, bone infections, abnormal growths and some metabolic bone changes. What it does not show well is soft tissue: a plain film gives little information about muscles, tendons, ligaments or the labrum. When those structures are the concern, your doctor may instead order an MRI scan, which images soft tissue in detail.
Why doctors order a hip X-ray

Common reasons for a hip X-ray include:
- Sudden pain or inability to bear weight after a fall or accident, where a fracture is suspected.
- Chronic hip or groin pain, stiffness or a grinding sensation that may suggest arthritis.
- Assessment of a suspected dislocation or a hip that feels unstable.
- Follow-up of a known condition, or a check on the position of a hip replacement or surgical hardware.
- Evaluation of hip development in children when a clinician has a specific concern.
Because the hip sits close to the spine and pelvis, symptoms sometimes overlap. Depending on where your pain is, your clinician might order a hip film, an L-spine X-ray, or a broader study of the pelvis to pinpoint the source.
How the exam works
An X-ray machine directs a small burst of radiation that passes through the body and records an image on a detector or film. For a hip study, the technologist positions you so the joint is captured clearly, often from more than one angle. A typical series includes a front-to-back view of the pelvis and a targeted view of the affected hip, sometimes with the leg rotated slightly to open up the joint.
You will be asked to hold still for a second or two while each image is taken, because motion blurs the picture. The whole exam usually takes about five to ten minutes.
How to prepare for a hip X-ray

A hip X-ray needs very little preparation. Helpful steps include:
- Wear loose, comfortable clothing; you may be asked to change into a gown so that zippers, buttons or metal do not obscure the bones.
- Remove jewelry, belts and other metal objects from the area being imaged.
- Tell the technologist if you are or might be pregnant, since the team will take extra care or consider alternatives.
- Mention any recent surgery, implants or hardware in the hip so the images can be interpreted correctly.
Tip: describe your pain clearly
Point to exactly where it hurts and describe what makes it better or worse before the exam. Groin pain, outer-hip pain and buttock pain can each point to different structures. Clear information helps the technologist choose the best views and helps the radiologist and your doctor interpret the images in context.
Radiation and safety
An X-ray uses only a very small dose of ionizing radiation, and no radiation remains in your body after the exam. While there is a slight theoretical increase in cancer risk from radiation exposure, the dose from a single hip X-ray is low, and for a clinically appropriate test the diagnostic benefit clearly outweighs that small risk.
Pregnancy is the main reason to pause and discuss options, because the pelvis and hip are close to the uterus. Always tell your care team if pregnancy is possible so they can weigh the timing, shielding and alternatives with you.
What to expect after the exam
There is no recovery time. You can return to normal activity immediately, and there are no lingering effects from the imaging itself. The radiologist reviews your images and sends a report to the ordering clinician, usually within a day or two, though urgent findings such as an obvious fracture are often flagged much faster.
If the X-ray does not fully explain your symptoms, your doctor may recommend additional imaging. For soft-tissue or subtle problems, that might be an MRI; for complex bony anatomy it might be a CT scan. This stepwise approach is normal and does not mean anything is wrong with the first test.
